Welcome to our office

· Thank you for choosing us as your health care provider.  We are committed to making your treatment with us as productive as possible.  The goal of our office is to extend the finest healthcare to our patients, and to render this care in a professional and compassionate manner.  In order to achieve this goal, we must deal with our patients responsibly.  To properly do so requires the adherence to certain policies.

 

What to bring to your appointment:

· Completed forms our office sent to you, any recent x-rays you have had taken, mouth appliance you may be suing under doctor’s direction, business cards of all care providers you have seen since your accident, business card from your attorney and all insurance cards and information.

 

Appointment Times/Cancellations:

· We see all patients on an appointment basis and ask that you call in advance so that we may reserve time for you.  We respect your time, making every effort to remain on schedule, and ask that you extend the same courtesy to us.  If you are unable to keep an appointment, we require 24-hour notice so that this time may be given to another patient.  As a courtesy, someone from our office will attempt to call you 48 hours in advance to confirm your appointment.

· Cancellation without 24 hours notice may result in a fee charged to your account.

· To arrange for an appointment in the TMJ Disorder Clinic, call (303) 421-2696.  We make every effort to accommodate emergencies.  Please inform us if you feel you have an emergency.

 

Payment Options:

· To reduce our administrative costs and keep our fees to you as low as possible, we ask that you pay your co-payment at the time you receive treatment. 
Acceptable forms of payment are:

· Cash or check

· Credit card:  MasterCard and Visa

· Pre-Determined and approved insurance

· Care Credit, a no interest finance plan at http://www.carecredit.com
